Visa Letter of Invitation Request Form

To create an Official Letter of Invitation from WOC 2011, please fill in the fields below. Please include complete and accurate information about you and your company, including passport number, gender and city of birth.  At the very bottom, make sure you agree that the information is accurate and then click "Generate Printable PDF" to shorten your wait time.

Incomplete information will not generate a letter that is valid for use in your visa interview.

PLEASE NOTE:  We do not mail letters to foreign countries, except under extraordinary circumstances.  To complete this process, we require a valid email address for correspondence, a shipping account number (if you require express shipping), and a complete destination mailing from you in order to fulfill your request for a Letter of Invitation.  Requests that do not include a valid email address will not receive an original Letter of Invitation.
